This form is meant to be used by BLE members who operate within the State of Louisiana due to the fact that the results are sent to the Louisiana State Legislative Board office only. Members whose originating terminal is outside of Louisiana but whose train operates into or through Louisiana should also use this form.

Please note; Sometimes the server does not accept the report and will give an error, go straight back to the form (with your 'back' button) and try re-submitting the form. You shouldn't have to fill in the form again, all of your info should be retained. This seems to happen when different  forms are submitted close together. Maybe the server just cannot keep up, I don't know. If the problem persists, drop me a note and I will try to figure it out.
