(I suppose example would be on FM2854 Low Boy Flat bed Bottomed out and was torn from tractor and fell across mainline)
Grade crossings with active warning devices such as lights and gates have signs attached to support pole with phone number to call and each crossing has assigned location number on sign. If sign is missing then check the big metal shed with crossing controls inside for phone number and location number stenciled on side. Recently when I found where gate had been hit and broken off ( And hit by train) it was not 3 minutes after I called it in before I heard dispatcher giving slow order to 15 MPH to train approaching and within 30 minutes signal maintainer hi railed up to repair (replace in this case) gate. Highway speed limit was 55MPH at this location.

BNSF also has signs at crossings with active warning devices with phone# and location # to call in malfunctions.
