We are seeking a self-motivated RN for the role of Community Care Manager to work on a full-time basis as part of our exceptional team providing at-home care in the South Western Sydney area.

HammondCare  is an independent Christian charity specialising in dementia care, palliative care, rehabilitation, specialist mental health for older people and other health and aged services.  The organisation provides these services through sub-acute hospitals, residential care and innovative community services in NSW and has a particular commitment to dementia care and research as well as to older people who are financially disadvantaged.

About you:

You are a Registered Nurse (List A) who has sound management skills as well as clinical skills.

You are seeking the opportunity to take on a leadership role with your own team of care workers.

You are competent in using computers for work.

 

About the role:

Part office-based, undertaking administrative and staff management duties, and part community-based, visiting clients in their own homes.  Driver’s licence required (package includes fully maintained car, with private use.  Friendly and supportive team environment.  Previous aged care or community experience is desirable but not essential, as comprehensive training will be provided.
