The Administration, Faculty
and Staff of the Ida B. Wells Community
Academy (hereinafter referred to as "the Academy") make and affirm the
following.
- The
Academy assures that
it will annually
provide the U.S. Secretary of Education and the Ohio Department of
Education
such information as may be required to determine if the community
school
is making satisfactory progress toward achieving the funded activities.
This assurance includes the Academy's participation in any federal or
state-funded
community school evaluations or studies.
- The
Academy assures that
it will cooperate
with the U.S. Secretary of Education and the Ohio Department of
Education
in evaluating the program being sponsored and/or assisted.
- The
Academy assures that
it will subscribe
to and maintain e-mail service to share and receive information with
other
community schools and the Ohio Department of Education.
- The
Academy assures that
it is nonsectarian
in its programs, admissions policies, and employment practices.
- The
Academy assures that
it complies
with the Age Discrimination Act of 1975, Title Vl of the Civil Rights
Act
of 1964, Title IX of the Education Amendments of 1972, section 504 of
the
Rehabilitation Act of 1973, and Part B of the Individuals with
Disabilities
Education Act.
- The
Academy assures that
it complies
with the same Federal and State audit requirements as do other public
schools
in the State.
- The
Academy assures that
it meets all
applicable Federal, State, and local health and safety requirements.
- The
Academy assures that
it operates
in accordance with the Ohio Revised Code.
- The
Academy assures that
it's admission
process will not discriminate against anyone regarding race, gender,
national
origin, color, disability, or age.
- The
Academy assures that
it will admit
students on the basis of a lottery, if more students apply than can be
accommodated.
- The
Academy assures that
it is in compliance
with all required federal assurances and, if funded, will cooperate
with
the Department in the development and submission of certain reports to
meet certain state and federal guidelines and requirements. The Academy
will work with and provide requested data to the successful
contractor(s)
in the evaluation of Ohio community schools.
- The
Academy assures that
it will maintain
appropriate fiscal and program records. The Academy also assures that
fiscal
audits of funds will be conducted annually as a part of its regular
audit
and will assure that auditors are aware of the Federal audit
requirements
contained in the Single Audit Act of 1984.
- The
Academy assures that
all monies
that may be awarded as a part of any grant, contract or extra-mural
funding
program will be spent or encumbered by the last day of the grant,
contract
or funding program. The Academy will also keep and maintain all
equipment
purchased with extra-mural funds in accordance with the requirements of
Federal and State laws and regulations.
