Before filling out this form, please make sure that you have 1) not already submitted any other horses for MAA approval this week, 2) have not submitted the horse you are about to submit for MAA approval to any other approval association this week, and 3) are willing to fork over $500 for inspection/approval (or not, depending on the horse!).
