Virtual Emergency Operations Center Log

This page is the daily log of operations in the Virtual Emergency Operations Center. The form used for this log is a virtual version of ICS 214.

The initial log entry for each incident will include the following data.

1. INCIDENT NAME: the common reference name of the incident.

2. DATE PREPARED: the date the log is opened.

3. TIME PREPARED: the time the log is opened.

4. UNIT NAME: Virtual EOC

5. UNIT LEADER: the assigned Virtual EOC Incident Commander for the incident.

6. OPERATIONAL PERIOD: the initial operational period covered by the log.

7. PERSONNEL ASSIGNED: initial personnel assigned to duty.

8. ACTIVITY LOG: the first action taken.

All entries are time and date stamped. As changes are made in any of lines 2 through 8 for the incident subsequent log entries will indicate the paragraph number referenced.
