Waiver Release and Covenant Not to Sue Form
This waiver must be signed before participation in any practice or race sponsored by Wasabi is allowed. READ CAREFULLY BEFORE SIGNING.
I the undersigned
(please print) __________________________________ hereby certify and declare that I am a participant inn a paddling program that requires periodic practices in water craft owned by Wasabi. In consideration for the privilege of use of the water craft for paddling activities, I do hereby personally, without reservation, waive, release, acquite, and forever discharge the water craft owner, officers of Wasabi, agents, and employees from any and all suits, claims, demands, or assertions of liability, injury to incorporeal interest, or other like damages occasioned by, arising from, or otherwise connected with my participation in the said program and do personally covenant that no action of law or suite in equity shall ever be brought against the sponsors, owners, or any of them, their officers, agents or employees, or the owner of the premises upon which the activities of the program are to be conducted on account of any matter hereinabove set forth.I further declare that I am aware of the activity contemplated and of the hazards connected therewith. I understand the necessity for close supervision of the activities of the program, and authorize the sponsors, owners, officers, and employees who may be in charge from time to time to direct and control my activities where necessary for my protection, and for the protection of all participants in this activity.
I further authorize the person in charge to secure any necessary emergency medical services for me, agree to pay for such services, and to hold the sponsors, and each of them, and their offices, agents, and employees harmless therefrom.
The provisions of this instrument shall be binding on my heirs, agents, successors, and signs.
