    • Order by Mail (with Check, Money Order):

    1. Fill out the order information below.
    2. Print out the page using your web browser's Print function.
    3. Mail the printed order form with payment amount quoted (Must get e-mail quote for this option. Other instructions will be e-mailed with confirmation.) You will be e-mailed an address and total of your order.
    Order by On-line Form (Payment must be received within 10 days of order confirmation.)
