Surveys and Questionnaires from the North Platte Public Schools


The following are surveys/questionnaires given to students and/or given to parents of students in the North Platte Public Schools. Note how these deal with attitudes/values of the students/families. After reading these, obtain a copy of the Grassley Amendment and the Hatch Amendment to find out what information you do not have to provide or what kinds of surveys you do not have to complete.


Given to my daughter in "Positive Lifestyles" class, Adams Middle School, 1996-1997. The two statements were paired with each other; one on the left and one on the right. Each student was to rate on a scale of 1 to 5 which statement described them best.

To express themselves more effectively, elementary guidance counselors should teach students:

1. various methods of expressing feelings

2. listening and speaking skills

To help them achieve, elementary guidance counselors should teach students:

1. how to plan and set goals

2. how to analyze, accomplish, and evaluate tasks for achieving a goal

To gain a more understanding of rights and responsibilities of self and others, elementary guidance counselors should teach students:

1. of the rights and responsibilities of themselves

2. of the rights and responsibilities of family members

3. of the rights and responsibilities of other students and adults in the school

4. of the rights and responsibilities of a world society

To handle conflicts with others, elementary counselors should teach students:

1. the problem solving process and how it may help them solve conflicts

2. when it is necessary to get help to solve a conflict and how to get that help

3. how to expect and cope with change

To learn good decision-making skills, elementary guidance counselors should teach students:

1. how values affect their lives

2. the decision-making process

3. how to use the decision-making process and how past decisions affect present decisions
