The following are surveys/questionnaires given to students and/or given to parents of students in the North Platte Public Schools. Note how these deal with attitudes/values of the students/families. After reading these, obtain a copy of the Grassley Amendment and the Hatch Amendment to find out what information you do not have to provide or what kinds of surveys you do not have to complete.
Given to my daughter in "Positive Lifestyles" class, Adams Middle School, 1996-1997. The two statements were paired with each other; one on the left and one on the right. Each student was to rate on a scale of 1 to 5 which statement described them best.
