Application Questionnaire
    The purpose of this questionnaire is to determine your knowledge and understanding of the field of educational technology.  Lack or prior experience will not prohibit admission into the cohort.  Please address each question below and limit responses to three (3) pages.  Place your name on each of the pages.

1.    Describe your knowledge of educational technology applications.

2.    Provide examples of technology-related professional development including
       workshops, conferences, and courses) you have attended.

3.    Provide examples of technology-related professional development you have
       provided to other educators.

4.    Provide examples of your use of technology in classroom instructional settings
       such as Josten's, PowerPoint, Publisher, etc.

5.    Explain your role in technology planning at school or district level.

6.    Provide additional information relating to other examples of leadership
       experiences in the field of educational technology.

7.    What career changes do you envision when you have completed the
       Educational Technology Leadership Program?
