Here's a sample of what my award looks like.
To qualify, your site must be Winnie-the-Pooh related (at least have a few pics of him or some of his friends). If you'd like to win my award, there are two other things you must do to qualify. First, you have to sign my guestbook (it's getting kinda lonely in there). Second, e-mail me with your site's URL, your e-mail address (so I can send you the award if you win it), and the name that you would like your award addressed to if you win (I will personalize your award for you). Also, make sure that you mention something about my award in the e-mail subject. I will try really hard to review all entries monthly, so the best site of the month will win. The winners will also get their site listed here on my site. And remember, if you don't win this month, you can always try the following month!!
