I carry a cards in my wallet and a medalion around my neck. This is what they look like:

I am no longer a diabetic, but I have of late started having very low blood sugar. I mean very low, blood glucose levels like 35. Fearing I might have a Hypoglycemic event and pass out, the only way I could think to give Emergency Response People a reason to check my blood sugar was to put "Diabetic" on the wallet card.
If you are a post-op WLS patient, you need to think about carrying some sort of device or card that will let Emergency Response People know something about your condition in case you are unable to speak for yourself.
These are just suggestions. Check with your doctor he might even have cards already designed that he will give you to carry. Below is a card that my doctor gave me to carry about the Vena Cava Filter Implant. I carry this card in my wallet with the other cards.
