A/5/12 Shirt Order Form

We are now taking orders for A/5/12 Golf Shirts. They will be 7oz Outer Banks brand, silver with an embroidered A/5/12 logo similar to the one on the top of this page. Basic price for shirts is $25.00 if picked up at reunion, or $30.00 if you want them shipped to you. There is also an additional charge of $2.00 for 2XL and $2.50 for 3XL. Please fill out and send the form below before Friday, April 14th, 2000 so we know how many shirts to order. You will recieve a return email with the $ amount and address to send your check to.
