Here is a picture of David's grave. David is buried in the Waltham Abbey, in Waltham, England. Here is what it says clearly in case you can't see the words in the picture too well.
At the top of his tombstone is a Jewish star, on either side is some writing in Hebrew. Sorry, but I don't know what that part says. Under the star and writing it reads:
David Stephen Rappaport
23rd Nov, 1951-2nd May, 1990
HE LOVED TO LIVE
HE MADE US LAUGH
His wit, talent and friendship gladdened
our hearts and challenged our minds
A VERY SPECIAL MAN
Deeply missed and loved by us all
May his spirit be a source of light and joy forever
