"Copy" of Pension Certificate

State of Indiana, County of Pike.

On this 7th day of January A. D. one thousand

nine hundred and thirteen, personally appeared

before me, a Notary Public, within and for

the county and State aforesaid,

George W. Brittingham who,

being duly sworn according to law,

declares he is 70 years of age

and a resident of Union R. F. D., county

of Pike & Gibson. State of Indiana;

and that he is the identical person

who was enrolled at Evansville State of Indiana

under the name of George W. Brittingham

on the 15th day of July 1862, as a Private, in

Co. G-65 Ind Infantry in the service of the

United States in the Civil War, and was HONORABLY

DISCHARGED at Greensboro, N.C.

on the 22nd day of June, 1865

He was not employed in the military or naval service

of the United States other than stated above.

(No other filled in) That his personal description

at enlistment is as follows:

Height: 5 feet 9 inches; complexion: Light;

Color of eyes: Grey; Color of Hair: Light;

that his occupation was Farmer; that he was born;

January 1st, 1843 at Pike County near Union, Indiana.

That his several places of residence since leaving the service

have been as follows: Pike & Gibson Counties, Post Office address

has always been Union, Ind or Union R.F.D. Ind.

He is a pensioner under certificate No. 651735. That he has

applied for pension under original number 651735.

That he makes this declaration for the purpose of being placed

on the pension roll of the United States under provisions

of the Act of May 11, 1912. That his post-office address

is Union R.F.D., county of Gibson, State of Indiana.
