Fantasy Form


Strolling along and enjoying yourself as you traverse the caves, you come upon a sheltered and musty room. The smoke from many candles causes a haze that stings your eyes. Your first immediate feeling is to leave, but you are silently drawn to the small room. Trekking deeper into the cave, you bump into a desk that has a sign that says, "Please wait, someone will help you as soon as possible." Following these directions, you sit on the ground and make yourself comfortable...

Out of the haze, a tall and greying man approaches you, and greets himself as a Lorewarden. After the two of you greet eachother, he asks if you would like to fill out a form. The wizard utters a magical phrase, and a form appears in your hands. He explains to you that this form is open to all public to fill out about Fantasy-related topics. He says that all information will be computed and processed. Any information will be acceptable and helpful.

After you agree to complete the form, the Lorewarden leads you to a small desk in the corner of the room and instructs you to fill it out to the best of your ability. Then he leaves you to fill out the form:
