Emergency Care forms
     You must have current Emergency Care forms on file in the Student Activity Office before participating in any practice or competition.

     The Emergency Care forms must:

  • Be signed by a parent, or legal guardian as designated on official school registration documents.
    • Forms signed by all others will not be accepted.

  • Be dated after May 15 of the previous school year.

     You may not practice with a Lee Lancer athletic team or be issued any equipment until two complete signed Emergency Care forms on file. One copy will be kept with the team equipment so it is readily available, while the other will be kept in the Student Activity Office.

Here is the Emergency Care form in PDF format. You will need to download the file, then open it in Adobe Acrobat Reader.

If you are required to submit a Emergency Care form to the school nurse for enrollment purposes, a photocopy of the document is satisfactory for athletic purposes and may be submitted to the Student Activity Office.

Sports participation necessitates physical risks. Requiring an Emergency Care form signed by a parent or guardian will permit us to contact you or your designated alternative contact should your student athlete be injured during practice or a game or match.
