Participation Insurance
     You are encouraged to participate in the FCPS  sponsored optional Participation Insurance program. Application  forms are available in the Student Activity  and School Office during the first two weeks of school.

     The Participation Insurance forms must:

• Be signed by a parent, or legal guardian as designated on official school registration documents.

Forms signed by all others will not be accepted.

• Be received in the school office by September 15 of each school year.

     Participation insurance is not required to participate in athletic team practices and games or matches.

     Participation insurance provides co-insurance that pays the difference between family health insurance coverage and the billed medical treatment and therapy.

     Sports participation exposes student athletes to physical risks and possible injury during practice, or during a game, match or meet.
