Physical Examinations Students and parents are reminded
athletic physical forms must be on file in the Lee Activity Office before you maycan participate in a try-out or practice with any team. |

You must have a current physical examination on file in the Student Activity Office before participating in any
practice or competition.
The physical examination must: |
|
|
|
- Be signed by a physician, MD or DO.
Forms signed by nurse practitioners, physicians assistants, or chiropractors will not be accepted.
- Be dated after May 15 of the previous school year.
You may not practice with a Lee Lancer athletic team or be issued any equipment until a complete signed physical is on file. Here is the
physical examination form in PDF format. You will need to download the file, then open and print it in Adobe Acrobat Reader
. Adobe Acrobat permits you to enter your personal data on to the form before printing it and visiting your physician.
If you are required to submit a physical to the school nurse for enrollment purposes, a photocopy of the document is satisfactory for athletic purposes and may be submitted to the Student
Activity Office. Sports participation necessitates physical risks. Requiring a physical by a qualified physician is but one way FCPS attempts to lower every student athlete's health and risk
of injury. Please Note The penalty to the school and sports team
for allowing a student athlete to participate in a contest without having a current, signed physical form on file is to forfeit all games or matches in which the student athlete participated. |
