     Complete this form for each athlete or relay team who meets a Northern Regional qualifying standard in a regular season meet and attach the dual meet entry card and send to the Regional Meet Director by Monday, February 8, 1999.  
     Any verification received after this deadline will NOT be accepted and the athlete or relay will be allowed to use that performance for a regional qualification.
