This page provides a few tips about some of the items requested on the " Check List "

            Biographic Data    :     This is the form 230Part -1, this is to be Completed and Returned
            for Visa Purposes       Immediately as stated on the " Check List Form "

            Passports               :     Must have or obtain a Valid Passport, for the Visa, you will need
                                                  the passport for Indentification at the Medical Examination as well.

            Photographs          :      Need Two Photos's 1-1/2 " X 1-1/2 " / full face view / like passport
                                                  photos but on thin paper. These you will need for the OF - 156. I chose
                                                 to use Passport Photos, and these were accepted !

            Medical                 :       You will have to Pay for the Medical Examination . Cash Only , and
            Examination                you will need your passport as identification. Also any Immunization
                                                 Records that you are able to obtain.      Book the Exam early .

            Birth Certificates   :   You will need a Certified Copy [ long form ] of your birth certificate .
                                                  It must include the Names Of Both Parents . Try to get extra copies
                                                  as you will need them at some point after moving to the U.S .

            Police Certificates  :    Please refer to the check list pages for further information on this one.
                                                 Also refer to OF -169 Suppliment .

            Divorce Decrees    :    You may need Copies of the divorce Decrees / death Certificates.
                                        These have to be certified , and Bear the Seal of the issuing authority .

            Evidence                :     This includes the I-134 and any Supporting Documentation that will
            of Support                    show that you will not become a public charge .     ( see OF - 167 )
