NOTE:  You are to arrange for a Medical Examination by one of the physicians listed in
              the enclosed instructions[ Canada ]. Please note that YOU must pay for the
              Medical Examination.  A Medical Examination is also required for each
              unmarried minor child./children who will accompany you to the United States.

           Cost :   The cost of the Medical Examination :  $ 210.00   CDN
                                          [ chicken pox ]   Immunization :  $   75.00   CDN            Only if needed !
                                          [ regular shots]   Immunization :      O.H.I.P.

            Tip: Bring your passport along to the Medical, you will need this, also, may help to call
                ahead to see what other identification they may require.

            Tip: Also note that the Medical Examinations is to be paid by Cash Only !
