The DQ happens only when you take in too much evil for your psyche to handle. This hasn't happened to Duncan before simply because he was always able to deal with what he had taken in. I can remember many an ep where at the end after the Q had been taken, Duncan would be either practicing martial arts or meditating or something.
The thing about DQ is this, the evil coming in gets incorporated with whatever evils you have within you FIRST then if that total is more than you can handle, you become evil. In a light quickening it happens the other way around.
