Copyright forms are simple to fill out. They may look intimidating at first but the instructions are very simple. Just read your instruction pamphlet that comes with the forms and read the forms before you start writing or typing on them. You will discover as you go along that the process is very easy and somewhat painless :) Basically all you need is your name, address, phone number, date of birth, and answer a few questions. Under "nature of authorship" you can enter author or co-author of lyrics or music, or lyricist, or songwriter whatever fits your role in writing the song best.

One quick tip is under the Title if you are doing a collection you need to title the collection and then list the separate song titles on the back of the Form CON or Continuation Form. In other words say your band is named "The Musicales" and you call your collection "Musicales Collection Number One" you enter that under title and then on your CON form on the back check mark section 1 and then enter the songtitles.
