When requesting birth, death, or marriage records from town or city clerks, always request that a photocopy of the original log entry be included as well. My experience has shown that in each case where I received a photocopy, there was information shown in the original entry which is not asked/provided on the certificate. The clerk's office may not always honor your request for the log entry, but when it does, you won't be sorry.
