REQUIREMENTS AND PROCEDURES:

1. NOTARIZATION

For Affidavits, Sworn Statements, Letters, Certification, etc.:

    1. personal appearance of person executing/signing the document
    2. identification (HKID or passport)
    3. extra copy of document for the Consulate’s file
For Affidavit for Parental Consent (for minors):
    1. personal appearance of parents (if both in Hong Kong/Macau)
    2. identification (HK/Macau ID or passport)
For Acknowledgment of writings (Re: Deeds, Power of Attorney, Real Estate Mortgage, etc…)
    1. personal appearance of person’s signing the document including the two witnesses
    2. identification (HK/Macau ID or passport)
    3. extra copy of the document for the Consulate’s file

2. AUTHENTICATION OF DOCUMENT

For documents issued in the Philippines and for use in HK/Macau:

    1. same document authenticated by proper authentication offices in the Philippines
    2. extra copy of said document for the Consulate’s file
 

For documents notarized in Hong Kong (for use in the Philippines)

    1. notarized document and photocopy
    2. proof of authority of the notary public
 

For documents issued in Macau (for use in the Philippines)

    1. same document notarized by a Hong Kong notary public and photocopy
    2. proof of authority of the notary public
 

For document issued by Hong Kong Government officials:

    1. original document and photocopy

3. CERTIFICATION FOR SEALING OF CASKET

 

Submit the following:

    1. death certificate
    2. embalming certificate
    3. mortuary certificate
-this should include the name, address, and contact number of

consignee, flight details and the information that the casket

contains nothing but the body of the deceased

    1. permit for removal of dead body from Hong Kong
    2. original and photocopy of the deceased’s passport

4. APPLICATION ON THE USE OF PHILIPPINE FLAG VESSEL

    1. duly accomplished form in five (5) sets (available at the notarial section)
    2. packing list (photocopy)
    3. bill of lading (photocopy)
    4. letter addressed to the Consul General

5. DOCUMENTARY REQUIREMENTS IN ESTABLISHING REGIONAL

HEADQUARTERS

Letter addressed to the Philippine Consulate General requesting

issuance of the certificate. The following should be attached to the letter:

    1. copy of Business Registration (HK)
    2. copy of Memorandum and Articles of Association
    3. company profile should include:
-Business Address

-Nature of Business

-Names of the Board of Directors

-List of Affiliates in Asia Pacific

    (d.)  Board Resolution authorizing   the opening of a Regional or Headquarters in the Philippines

6. OTHERS

Those who wish to bring pets to the Philippines (e.g. dogs, cats, birds) can get the guidelines/procedures at the notarials counter.
