VI. PAYMENT FOR FEES:
1. No fee is required for reservation
2. Confirmation of the reservation shall be made (address to the CME Chief Program Officer Ivan N. Villespin,
M.D., M.B.A.) anytime but not later than 2 weeks before the date of the activity, at which time a deposit of 50% of the
minimum projected basic fees is required.
3. Cancellation of the reservation with in 2 weeks of the date will automatically forfeit the deposit. Postponements
to a later date will be arranged depending on the availability of the CME Auditorium
4. Full payment of the balance shall be made not later than 2 weeks after the activity, other wise, a 3%
of interest per month will be added until payment is completed.
