Moon Prism Power Make-Up!
Usagi first used this in Act 1 (or Volume 1 in the Kodansha BSSM comic), and it last appeared in Act 14 (Volume 4). Usagi used the brooch she got from Luna, which is more adorned than in the anime, to transform into Sailormoon; she yelled the phrase and in a flash of light she became Sailormoon, and she struck a pose. Also, when she transformed she gained a mask, much like Sailor V's. They helped her with honesty and vision and truth.
Moon Power!
Usagi first used this in Act 2 (Volume 1), and it last appeared in Act 24 (Volume 7). Usagi said the phrase and what she wanted to transform to. She threw her pen up--it was a pen she won from the SailorV game when Ami-chan was playing it for the first time. (Ami got a high score, and her henshin pen popped out; Usagi seemed jealous and knocked the game, and her transformation pen popped out.) Finally she transformed into whatever she wanted to transform to and posed. (V-Chan also had a pen similar to Usagi's, and she used it often in the Sailor V manga. But she used it rarely in the Sailormoon manga, the first time being in Act 26, when she changed into an Infinity Academy student to sneak into a concert.)
In Act 2 (Volume 1) Usagi transformed into a doctor.
In Act 3 (Volume 1) Usagi transformed into a stewardess.
In Act 4 (Volume 1) Usagi transformed into a beautiful princess.
In Act 5 (Volume 1) Usagi transformed into a handsome groom.
In Act 24 (Volume 7) Usagi transformed into an Infinity Academy student.
Moon Crystal Power Make-Up!
Usagi first used this in Act 13 (Volume 4) and it last appeared in Act 23 (Volume 7) . Usagi used a newer compact which she got from Luna, as her old one broke in the battle with Metallia. In a flash of light Usagi transformed into Sailormoon, her brooch changing again, of course.
Moon Cosmic Power Make-Up!
Usagi first used this in Act 24 (Volume 7) and it last appeared in "The Lover of Princess Kaguya" (Volume 11); Usagi said the phrase in Act 34 (Volume 12), but nothing happened. Usagi used a newly-conformed compact with the SIlver Crystal and a heart shaped brooch (made by the future Queen Serenity) and she shouted the phrase, and in a brilliant flash of light she turned into Sailormoon. (Her fuku changed a little too.)
Crisis Make-Up!
Sailormoon first used this in Act 30 (Volume 9) and it last appeared in Act 33 (Volume 10). Using the power of the holy chalice in a crisis, Sailormoon said the phrase and powered up into Supersailormoon. She needed the energy of the other senshi though. Her costume changed a lot like in the anime; she had a multi-colored skirt and she had a heart above her skirt area. However, she already had barrettes since her first transformation.
Moon Crisis Make-Up!
Usagi first used this in Act 34 (Volume 12), and it last appeared in Act 40 (Volume 14). Usagi got this power from within herself, as did Chibiusa. She said the phrase and both Usagi and herself transformed, into Supersailormoon and Supersailorchibimoon, respectively. A double henshin. They couldn't use the holy chalice anymore, because the three outer senshi disappeared, and the nine needed to combine their powers to make the holy chalice work.
Silver Moon Crystal Power Make-Up!
Usagi first used this in 43 (Volume 16) and it last appeared in Act 46 (Volume 17). Usagi her used conformed ginzuishou and said the phrase, and she transformed into Eternalsailormoon. Later she gained wings on her suit, via Princess Kakyuu, only to get them ripped off by the evil senshi (under Galaxia's influence.)
Moon Frisbee!
Usagi first used this in Act 1 (Volume 1), last appeared in Act 14 (Volume 4). Sailormoon said the phrase and took off her tiara, which conformed into a frisbee; it was a circle with a hold in the middle, kind of like a crescent moon with the tips attached. She threw it at an enemy, in most cases killing it.
Moon Twilight Flash!
Sailormoon first used this in Act 4 (Volume 1) and it last appeared in Act 7 (Volume 2). Sailormoon used her new tiara (her previous one got burned up with the fight with Jedite.) She said the phrase and touched her tiara, which flashed, and moonlight was emitted, utterly killing the enemy.
Moon Healing Escalation!
Sailormoon first used this in Act 7 (Volume 2) of the manga, and it last appeared in Act 13 (Volume 4). Actually, it first appeared in Act 6, but was nameless. Well, Sailormoon used her new weapon, the moon stick, which was bestowed to her by Luna. She said the phrase and the moon stick healed people in trances, by giving them good energy.
Moon Princess Halation!
Sailormoon first used this in Act 14 (Volume 4) of the manga, and it last appeared in Act 23 (Volume 7). Sailormoon used Queen Serenity's rod in this attack. It appeared much in the same fashion as the Spiral Moon Heart Rod appeared in the anime in the third season. Well, Sailormoon said the phrase, and a light bursted from the rod, killing the enemy.
Moon Spiral Heart Attack!
Sailormoon first used this in Act 25 (Volume 8) and it last appeared in Act 33 (Volume 10). Sailormoon used the new rod given to her by Chibiusa, a present from Neo-Queen Serenity. It was more ornate and it had a heart at the end of it, and it looked exactly like the rod from the anime. She used this attack with Neo-Queen Serenity's power though. She said the phrase, and much like Moon Princess Halation, the enemy died, but it exploded instead.
Rainbow Moon Heart Ache!
Supersailormoon first used this in Act 30 (Volume 9), and it last apeparred in Act 33 (Volume 10). Supersailormoon said the phrase and held her moon rod at the enemy, vanquishing them. It was the same rod used in "Moon Spiral Heart Attack."
Rainbow Moon Double Heart Ache!
Supersailomoon first used this in Act 33 (Volume 10) of the manga, and it only appeared once. Both Supersailormoon and Supersailorchibimoon took their moon rods as they said the phrase, and they joined their energy to blast the monster.
Moon Gorgeous Meditation!
Supersailormoon first used this in Act 34 (Volume 12) and it last appeared in Act 41 (Volume 15). Like the double henshin "Moon Crisis Make-Up" came a double attack. Supersailormoon used her special Moon Kaleidscope and said the phrase along with Supersailorchibimoon, as they sicked their weapons at the enemy, and the enemy got destroyed.
Starlight Honeymoon! Therapy Kiss!
Eternalsailormoon first used this in Act 42 (Volume 15), and it last appeared in Act 50 (Volume 18). Eternalsailormoon used her new rod in this attack, and the first time she used it was to abolish Nepherenia. She said the phrase and light radiated from her rod, pure light, destroying evil.
Starlight Honeymoon! Double Therapy Kiss!
Eternalsailormoon first used this in Chibiusa's Picture Diary (Volume 17) and she only used it once. It was a double attack with Chibimoon. The two held out their scepters, said the phrase, and the enemy vanished; the enemy the only time they used it was a ghost possessing Hotaru-chan.
Silver Moon Crystal Power Therapy Kiss!
Eternalsailormoon first used this in Act 50 (Volume 18), last appeared in Act 51 (Volume 18.). This power was given to Eternalsailormoon by Sailorchibichibi, when the evil senshi, influenced by Galaxia, snapped her scepter in two. Sailorchibichibi started to pray and her scepter reappeared, with a new power.
