How to Order Death Certificates

Photocopies of Kentucky death certificates for the years 1911 thru 1955

You'll receive a photocopy taken from microfilm that has been made public (privacy restrictions on death certificates expire after 50 years). It will be as shown but will not be a certified copy like those received from the Department of Vital Statistics

How to Order
The cost for this service is $3.50 per requested certificate. To order, send:

DEATH VOLUME Number (from the online index - very important!)
A self-addressed, stamped envelope (1-5 certs.), if you order more than 5 then a large vanilla envelope would be best!
  (Rule-of-thumb: 1 stamp for every 5 certificates in a white business envelope and 2 stamps for every 5 certificates in a large vanilla envelope)
A check for $3.50 per certificate, payable to me at the address below.
  (One check covering multiple certificates is acceptable, and preferred)
