Name Change

Purpose

1. Request for name change is submitted upon legal approval of name change (i.e.: marriage or divorce).
2. Minor changes involving spelling, correction of transposed name or the addition or deletion of Jr., Sr., III, these minor changes can be made on a DA 4187 without documentation.

Approving Authority
PSB Commander.

1. Legal reasons for name change (Marriage Certificate, Divorce Decree, Court Order, etc).
